Hate filling out paper work in waiting rooms? Print out our patient/medical history questionaire and fill it out at home! Then bring it with you to your scheduled appointment. It will speed up the check-in process and get you in to see the Doctor faster!

Feedback from our patients is the best way for us to learn what we are doing right and where we need to improve.  If you have been in for an eye exam at either of our locations recently we would appreciate your feedback.  You can either print out the form and mail it in or drop it off at either office.  To make it even easier you can copy the survey into an email and send it to tammy@insighteyecarecenter.net
